As part of the South Side Healthy Community Organization (SSHCO), which is comprised of 13 healthcare organizations including safety-net hospitals, health systems, and federally qualified health centers, the goal is to better connect existing health organizations, increase access to care, address some of the most challenging health issues, and ultimately build health equity and ensure stronger, healthier communities across the South Side of Chicago.
We are hiring full-time, culturally competent community health workers (CHW) to work at one of three sites:
- Friend Family Health Center, located at 800 East 55th Street with clinic locations at 4802 S. Ashland Avenue, 25 W. 47th Street, 5635 S. Pulaski Road, 5843 S. Western Avenue, 1522 E. 63rd Street, and 800 E. 55th Street
- Near North Health, with clinic locations at 4829 S. Cottage Grove Avenue and 4259 S. Berkeley Avenue
- St. Bernard Hospital, located at 326 W. 64th Street
As a Community Health Worker, you will provide health education, disease management education, health care and social service navigation services, and/or case management services to patients/community residents.

In addition to a passion for and dedication to working with community members and providers, we require:
- High school diploma or equivalent
- One year of experience in CHW, healthcare, or related experience, preferred
- If you live in a South Side community, that’s a plus!
- Demonstrated commitment to racial and health equity
- A valid driver’s license and own a car
- Proficiency in Microsoft Word, Excel, Access, and PowerPoint preferred
